1. Schedule your Appointment

Fill out a simple online form to tell us what day/times work well for you. We’ll then contact you to confirm a specific day and time.

Please know MHS participates with most insurance plans, including but not limited to, Blue Cross/Blue Shield, Medicare, Medicaid, as well as many commercial and managed care plans.

Checking your coverage with your insurance company prior to requesting an appointment with an MHS provider is recommended. If MHS has a contract with your insurance plan, you will be asked to pay for any out-of-pocket costs, such as copays, coinsurances, deductibles, as well as any non-covered services. If your insurance company does not participate with MHS, we will send your insurance plan a bill as a courtesy, but any amounts your insurance company does not pay will be your responsibility.

3. Rescheduling an Appointment

If you need to cancel or reschedule your appointment call us at 810.235.2004. Please contact us 48 hours prior to your appointment.

4. Updating your Health Information

Once you are a patient you can access your health care information at your leisure to update details or view some of your patient records.
