We are committed to providing all patients with the best possible care and service. That’s why no one will be denied services due to inability to pay, and we use a sliding fee scale at our Longway Blvd. clinic to ensure access to healthcare for everyone.
1. Schedule your Appointment
Fill out a simple online form to tell us what day/times work well for you. We’ll then contact you to confirm a specific day and time.
Please know MHS participates with most insurance plans, including but not limited to, Blue Cross/Blue Shield, Medicare, Medicaid, as well as many commercial and managed care plans.
Checking your coverage with your insurance company prior to requesting an appointment with an MHS provider is recommended. If MHS has a contract with your insurance plan, you will be asked to pay for any out-of-pocket costs, such as copays, coinsurances, deductibles, as well as any non-covered services. If your insurance company does not participate with MHS, we will send your insurance plan a bill as a courtesy, but any amounts your insurance company does not pay will be your responsibility.
